Pre-requisite: Internal Medicine or Family Medicine residents who have completed PGY-1 and the first half of PGY-2

Educational Objective

To provide Internal Medicine or Family Medicine senior residents (mid-PGY-2 or PGY-3) with a 2 week elective on the Franklin Memorial Hospital’s Hospitalist Service practicing rural inpatient care. Franklin Memorial is a 65-bed hospital in Farmington and is a member of MaineHealth.

Experiences and Procedures

  • Work closely with a hospitalist providing evidence-based quality care in an acute care in a rural hospital setting including managing patients both on the ward as well as in our 4 bed intensive care unit 
  • Discussing admission requests with the emergency department physician to determine if admission is appropriate, level of care given the patient’s condition, and any additional immediate care that should be implemented 
  • Perform procedures as needed with attending supervision including paracentesis, thoracentesis, lumbar puncture, as well as potential for intubation, and central line and arterial line placement  
  • Participate in the interdisciplinary care rounds by communicating patient’s progress, addressing RN needs/concerns, and providing an estimated discharge date for care management to assist with disposition  
  • Determining appropriate diagnostic and therapeutic interventions – and coordinating patient care at tertiary care centers when not available in-house  
  • Complete hospital medicine consult requests for the ED and other specialty groups as requested  
  • Assessing when a consult is needed, formulating a clinical question and discussing the salient patient background information with the consultant

Housing

On-site housing is offered at no cost to the learner at the Dixon House, when space is available. If space is not available at the Dixon House, then the resident may request funds from MERGE to reimburse for housing in Farmington, per the MERGE housing policy.
