Pre-requisite: Open to Family Medicine residents at the PGY2 level or above

Duration for Elective: 4 weeks split as 2 weeks in Jackman and 2 weeks in Bangor

Educational Objective

To provide the GME learner with clinical experience on the delivery of unscheduled urgent and emergency care in the rural non-emergency department setting including as both the originating site and remote site physician for telemedicine services. 


For the first 2 weeks, the rotation schedule will be primary care / urgent care clinic at the Jackman Community Health Center Monday, Tuesday, Wednesday and Thursday 0730-1730 and on call with the Critical Access Integrated Paramedic (CAIP) Tuesday Night 1730–0730 and Thursday night 1730 through Monday 0730 with 10-15 hours of clinical service delivery during that period. 

 

For the second 2 weeks, the rotation schedule will be emergency department shifts 0700-1900 Friday – Sunday with primary responsibility as the remote site physician for Jackman CAIP calls. The participant will also select one 8-hour shift per week in the St. Joseph Internal Medicine Unscheduled Services clinic. 

Experiences and Procedures

  • Provide routine, urgent, and emergency medical care in the rural primary care clinic setting. 
  • Participate in scheduled community paramedicine visits in a rural setting without home health resources. 
  • Participate in emergency medical services 911 responses. 
  • Participate in facilitated tele-urgent care visits with the Critical Access Integrated Paramedic at the originating site. 
  • Provide emergency department care in a community hospital emergency department. 
  • Provide facilitated tele-urgent care visits with the Critical Access Integrated Paramedic as the remote site physician. 
  • Provide unscheduled urgent care services in the community primary care setting. 
  • Receive orientation to and training in the use of telehealth platforms and peripherals as an originating site and remote site user. 

Housing

Jackman Community Health Center offers on-site housing as part of its EMS services. Learners have a private bedroom with a TV and a private bathroom. The kitchen and living area is a shared space with the paramedics. The housing and EMS integration is critical to this health center’s unique operation.

 

St. Joseph Hospital does not offer learner housing in Bangor. If a resident needs housing reimbursement, they may seek funds from the MERGE Collaborative under the parameters of the MERGE Collaborative Housing Reimbursement Policy.
